The breach occurred when an electronic file containing sensitive personal information was accessed by unauthorized individuals. Upon detection, the company took immediate action to secure its systems and launched a forensic investigation. The investigation confirmed that the data exfiltrated included names, email addresses, health insurance member and group numbers.
Affected individuals were offered 12 months of free credit monitoring and identity theft protection services. The company also implemented security measures to prevent future breaches.
